Repeat prescriptions are issued with a counterfoil on the right hand side. You just need to tick the items you need and drop it into the surgery. Alternatively you may post, fax or email it to firstname.lastname@example.org (clinical queries should not be sent via this address, it is a no reply service and is not looked at by clinical staff).
We do not accept telephone requests to avoid any mistakes.
Please submit a separate written request for any additional items you need rather than adding them to the counterfoil. We need at least 48 hours' notice (collection guide times are displayed in reception).
If you want us to post the prescription back to you, please supply a stamped addressed envelope. Some local pharmacies collect from us so your prescription can be sent to them direct. If you lose the counterfoil, request forms are available in reception. You will be asked to come in for a check up at sensible intervals in line with policy. Batch prescribing enables patients to collect regular prescriptions from a pharmacy of their choice without coming into the surgery. Only patients who are stable on their medication are suitable and your doctor will inform you if this is an option for you.
