Register with our practice

We are welcoming new patients who live in our practice catchment area. To register please complete the online registration form using the below link. We cannot accept registrations for patients who live outside of our catchment area so please ensure you check this below before completing our registration forms.

If you are registering a child under 6, please provide us with their immunisation records either by emailing them across to us on swlicb.twickenhamparksurgery@nhs.net or by dropping it into reception.

If you are registered with another practice in the local area, we will ask you to meet with one of our GP partners prior to registration.

Catchment Area (Practice Boundary)

Named GP

Once registered with the practice, you will be assigned a named GP. However, as we are a training practice, we have GP’s who rotate on 6–12-month placements. This means that appointment availability may vary. Rest assured, our computerised medical records ensure that any clinician you see will have full access to your details, allowing for seamless and informed care.

Temporary Registration

If you’re staying in our area temporarily and require medical care, you can register with us as a temporary patient. This allows you to access GP services without permanently changing your registered practice.

Temporary registration is available for individuals who will be in the area for more than 24 hours but less than 3 months. After this period, you’ll need to re-register or return to your permanent GP.

To register, simply complete the Temporary Registration Form and either email it to us on swlicb.twickenhamparksurgery@nhs.net or bring it into the practice. Our team will guide you through the process and ensure you receive the care you need during your stay.
