1. Check Your Destination’s Requirements
   Visit NaTHNaC to find out which vaccinations are recommended for your destination.
2. Complete the below form
   Please complete the below form and we will compare your responses with your vaccination history and contact you to arrange any additional vaccinations you may need. If you cannot complete the form please send us a list of the required vaccinations to swlicb.twickenhamparksurgery@nhs.net.
3. Book in Advance
   Some vaccinations require multiple doses over several weeks, so please allow plenty of time before your departure to complete the necessary courses.
4. Charges & NHS Coverage
   Certain travel vaccinations are not covered by the NHS and may incur a charge. Full details of costs are available at reception.
